We are in the process of reviewing our review system and obtaining feedback from Managers by way of a questionnaire to get their input into improvements for 2009.

I am currently designing the questionnaire and thinking about the types of rating scales that may be appropriate.

Would anyone be able to share with me past experiences of current best practice (or examples) for effective rating scales for performance review?

Also any suggestions on questions I should include within the questionnaire (incase I’ve missed anything obvious) would be greatly received!
