If you have a life long medical condition do you need to inform new employers of this at the start of your employment or is it sufficient to complete the medical questionaire given stating that you are taking life long medication if this is all that has been asked of you?

In view of this, and worsening of a condition, how do the employer and employee stand when the medical condition is disclosed at a later date during employment and after the probationary period?
