I am currently running a project to research the different mechanisms for collecting feedback/360 feedback. We have an online tool which is utilised by some within the organisation but it would appear that others want something which is less formal and rigid.

Does anyone have any thoughts on a less formal mechanism and/or any templates which they have used to this end?
